The Pivotal Flaps course is designed to equip medical professionals with the essential skills to perform skin flap surgeries, particularly for skin cancer treatment. The course, presented by Dr. Francis Tan, is structured into four comprehensive units, each covering critical aspects of pivotal flap procedures, with practical insights and detailed demonstrations.

Unit 1: Theory and Principles This unit introduces the foundational concepts of skin flap surgery, including the different types of skin flaps (random and axial flaps), and the core principles of designing a flap. Participants will gain a deep understanding of how to evaluate and plan flap surgeries based on patient factors and defect size, ensuring the best cosmetic and functional outcomes. The module emphasises careful consideration of tension, blood supply, and the importance of undermining to avoid complications​.

Unit 2: Design and Examples The second unit focuses on the design and mechanics of the rotation flap, detailing the geometry and angles involved in planning the arc of rotation. Various case studies and clinical examples illustrate the use of rotation flaps in different anatomical locations. This unit highlights the importance of understanding skin tension lines and the potential complications such as dog ears and tension points during flap placement.

Unit 3: Rotation Flap – Triangular Defect Technique This unit offers a step-by-step guide to performing a rotation flap repair for triangular defects. Participants will learn how to calculate the necessary arc and incisions, followed by practical insights into suturing techniques and dog ear management. The instructional videos and images focus on achieving smooth closure and minimising tension during the repair process​.

Unit 4: Rotation Flap – Circular Defect Technique The final unit covers the repair of circular defects using a rotation flap. Detailed instructions are provided on selecting the pivot point, designing the arc, and incising the tissue to match the defect's depth. This unit includes practical tips on tissue handling, suturing, and managing redundant tissue or dog ears to ensure a clean and functional closure​.

By the end of this course, participants will be proficient in the planning and execution of pivotal flap surgeries, allowing them to effectively manage complex skin defects while minimising complications and improving patient outcomes.

  1. Plan dog ear repairs safely for use in clinical settings.
  2. Determine the steps required to perform rotation flaps.
  3. Conduct surgical procedures in a simulated setting including various suturing techniques, elliptical excisions, rotation flaps, advancement flaps and full thickness grafts.
  4. Judge when to refer patients to others.

ABOUT THE PRESENTER

Dr Francis Tan

This course is presented by Dr Francis Tan: MBBS, MMed (Skin Cancer).

He graduated from The University of New South Wales in 1986. He worked in a variety of general practice surgeries throughout Australia, including clinics in Sydney, Tweed Heads, the Gold Coast, Brisbane and Cairns. He obtained a Masters of Medicine from The University of Queensland in 2014 and holds an impressive portfolio of university-certified Professional Diplomas in Skin Cancer Medicine and related fields. He is particularly interested in using dermatoscopy to detect melanoma in situ and performing skin cancer surgery to the best of his ability for the benefit of his patients. Dr Tan has over 36 years of experience in General Practice. From 1997, he has been in full time skin cancer practice.
